Hospice at Home Receives National Reaccreditation

chap-logoHospice at Home, a program of Caring Circle, a Lakeland Health Affiliate, recently received reaccreditation with deemed status from the Community Health Accreditation Program, Inc. (CHAP), the leading accrediting agency nationally for hospice and home health organizations. CHAP accreditation demonstrates that Hospice at Home meets the industry’s highest nationally recognized standards. Rigorous evaluation by CHAP focuses on structure and function, quality of care, human and financial resources, and long-term viability.
