Better health, it takes a village

Our community plays a major role in our overall health and wellness, influencing health outcomes and inequities. The conditions in which people are born, grow, work, live, and age are referred to as the Social Determinants of Health (SDoH) and they, as well as other forces and systems, shape our daily lives.  

Access to health care, child/parenting resources (like diapers and wipes), legal aid, financial planning, and assistance with food, insurance enrollment, and transportation services, are just some of the needs that when left unmet, affects our ability to be healthy.

Addressing SDoH are necessary to improve health and reduce longstanding inequities, with help needed from all sectors within our community, not just health care. Staff at the Corewell Health Center for Wellness partner and collaborate with community-based agencies and organizations to impact a variety of these social risk factors. Our community health workers provide navigation services to connect you to the local resources you need to achieve better health through person-centered resolution and care.
