Community Health Needs Assessment

A Better Place to Live and Grow

Community of people holding hands

Where and how we live, such as whether we have access to nutritious food, economic opportunities, quality schools, and safe neighborhoods, are vital to our health.

It takes many and diverse partners to ensure that our communities are healthy for everyone. The Population Health team at Spectrum Health Lakeland is committed to engaging all members of our community in our efforts to ensure good health for everyone.

We invite you to review our 2016 and 2019 Community Health Needs Assessments below and partner with us in our Implementation Strategies created to improve community health.

Want to get in involved? Please call (269) 556-2808 or send an e-mail to populationhealth@lakelandhealth.org.
