Remarks

This test can be ordered in the Epic system by the provider. This test order is a shell record with no CPT. The patient is direct billed by Cologuard, Exact Sciences Laboratories.

(1) The nurse/MA sees the order, pulls out the pre-printed Cologuard order and has both the patient and provider sign the hard copy. (2) A signed copy is scanned to the encounter. (3) A signed copy is faxed to Cologuard. (4) After receiving the order, Cologuard (Exact Sciences) will contact the patient and send a collection box to the patient’s confirmed home address and discuss insurance coverage. (5) Patient collects the specimen and returns by parcel service directly to Cologuard (Exact Sciences). (6) Results sent to the ordering provider. (7) Provider office staff scan the result to the Cologuard order
