Advance Care Planning Workshop Add to Calendar

If you’re over 18, it’s important to name someone you trust to speak for you in a medical emergency and state what kind of medical treatment you like to receive if you can’t speak for yourself. You need to have a plan in writing to share with your family and healthcare providers. Join us for a free workshop that will answer questions and assist with:

• Developing and completing your Healthcare Power of Attorney/Advance Directive
• Witnessing of signatures
• Adding your document to your medical record so it’s available to all Corewell Health hospitals and providers in case of emergency
• Having copies made for your advocates.
