Lakeland Medical Practices Earn PCMH Designation

Lakeland Family Medicine, Niles and Southwestern Medical Clinic, Bridgman, are now among a growing number of healthcare practices in Michigan who are improving healthcare quality by adopting the Patient-Centered Medical Home (PCMH) model of care. The practices are the first among a four-site pilot project sponsored by Lakeland’s Physician Hospital Organization, Lakeland Care, Inc., to earn a Level Three Patient-Centered Medical Home Designation from the National Committee for Quality Assurance (NCQA).

The PCMH model of care is a new approach to healthcare that provides a way to give patients better, more personalized team-based care. According to NCQA, medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care.

“Obtaining the designation of being a Patient-Centered Medical Home is so important for our office, as well as Lakeland, with the changing landscape of healthcare,” said Lindsay Iwan, Manager of Lakeland Family Medicine, Niles. “It was truly a team effort that each associate and physician worked very hard to help achieve. Our designation sends a strong message to patients and the community that our services are driven and focused on them.”

Among the other sites piloting the project, the Southwestern Medical Clinic practices located at 2002 South 11th Street in Niles and at 5515 Cleveland Avenue in Stevensville plan to submit for designation in the fall of 2014.
