The annual influenza vaccination program beganTuesday, September 7. Team members must get a flu shot by Friday, December 3, in order to meet the requirement.

Flu shots are free for all team members and spouses, providers, residents, students, volunteers, and retirees of Lakeland.

Employee Health is providing open flu shot clinics at several Lakeland locations. Please get vaccinated immediately. Together, we can keep our patients, our community, and each other healthy and safe. 

Things to keep in mind:

  • Egg-free vaccinations are available
  • High dose vaccinations will be offered to team members who are 65 years old and up
  • Anyone who receives the flu vaccine elsewhere must have documentation on file with Employee Health by December 3
  • When flu activity has reached a determined threshold and been announced, exempt staff will be required to wear a surgical mask at all times within six feet of any patient

All medical and religious exemptions for flu must have been documented and submitted by October 30. The exemption process changed this year.  

If you are requesting a medical exemption, please follow these steps:  

  1. Complete the Medical Exemption from Required Vaccine form prior to step 2; this needs to be signed by you and your physician.  
  2. Click here to access the OnBase exemption application.  
  3. Follow the steps in OnBase to complete the exemption application. Attach your completed medical exemption form.
