Appointment Cancellations
We think it’s important to offer appointments in a timely manner so you can receive care when you need it. Call the practice as soon as possible if you must cancel your appointment, so that time can be filled by another patient in need of care. If you provide less than 24 hours’ notice, it may be documented as a “no show.”
We understand that there are sometimes unavoidable emergencies and busy schedules that interfere with scheduled appointments. Please let us know if you can’t make it to your appointment. Three “no shows” or missed appointments in a six-month period may result in dismissal from the practice.
In order to successfully achieve the standard of care that Lakeland promotes, a positive provider-patient relationship must exist. If there are any barriers to achieving and maintaining this relationship, the provider and management have the right to determine if the patient is a good fit for the practice and may dismiss the patient. Any incident of abusive or rude behavior towards our providers or associates may also result in a dismissal. Pain contracts between provider-patient are important to the plan of care set-up for patients; if the contract is broken it is grounds for dismissal from the practice.
Patients who have been dismissed from our practice will receive 30 days of emergency care services. Medical records will be transferred to a new doctor upon the patient’s request.
Please bring photo identification and your current insurance card to every appointment.
