The Population Health team works to address the needs of our most under-served communities.
These needs represent social factors or social determinants of health (SDoH) which are the conditions in which people are born, live, work, play, and age.
The distribution of SDoH shape a community’s access to state-of-the-art education, structurally sound homes, transportation, and quality, nutritious food; the availability of living wage jobs, cultural, and physical or recreational opportunities; clean air and water; a sense of safety and security; and embeddedness in dense, resourceful social networks – all of which are critical to good health.
