Medical Records Information
Patients or their authorized representative may obtain copies of their medical record, or have copies sent to another medical facility, by following the instructions below.
How to request your Medical Records
Your request must be in writing and include the completed form, Authorization for Release of Health Information, before we can provide copies of health records.
If you have a MyChart account, you will be able to view your medical records online.
- Specify what information you need
Although many patients request their entire medical record, our experience has shown a summary of information from documents such as progress notes, lab results or diagnostic testing for a specific time period is usually sufficient. If the exact dates of service are not indicated, your most recent documents will be copied. Please feel free to call us if you have any questions.
- Personal Representatives must also include a copy of any court paperwork
Is there a fee to obtain copies of my medical records?
If you would like your medical records mailed to a physician or medical facility, there is no charge to you.
If you are requesting copies for yourself, there will be a charge of $1.00 per page for up to 20 pages, $0.50 for 21-50 pages, and $0.20 thereafter. (These fees are set by the State of Michigan). An invoice will be mailed to you with your health records; you are responsible for paying the invoice immediately upon receipt.
If you have Medicaid or Medicare you are entitled to 1 free copy of your records in your lifetime.
For attorneys, law offices, etc. requesting records, a $20.00 processing fee applies in addition to the per page fees.
How long does it take to receive my records?
Please allow up to 30 days to receive your health records.
