Who we serve

HouseCalls may be an option for those who have serious or chronic conditions and who are unable to travel to a physician’s office or clinic for care. Some examples include those recently released from the hospital or other inpatient facility, those with balance or mobility issues, and those who may have behavioral health conditions.

Request Information

Bringing expert primary medical care directly to you

Our skilled physicians and providers see patients in their own homes, adult foster homes, and assisted living facilities. HouseCalls does not require referrals or pre-authorizations from other physicians to begin medical care.

Should supplementary care be needed, HouseCalls can coordinate additional Lakeland Health resources, including Homecare services, palliative care, and Hospice at Home services. We will also arrange in-home diagnostic testing including blood work, mobile x-ray, or portable electrocardiograms (EKG) which are used to measure heart activity.

How does a HouseCalls visit work?

We will send a licensed clinician (nurse practitioner, physician assistant or physician) directly to your home to discuss your healthcare needs and create a plan specifically for you. You pick the date and time that will work best for your schedule.

Who can have a HouseCalls visit?

Check with your health plan to see if HouseCalls is an option for you or call our office with any questions. HouseCalls accepts Medicare and Medicaid, and will bill secondary insurance for any co-pays, if needed. We also work with most private insurances and will verify benefits and let you know if you will be responsible for any charges.

What happens during a HouseCalls visit?

During this visit a licensed clinician will:

  • Review your medical history
  • Discuss any health concerns you may have and provide answers and information about them
  • Perform a wellness check-up that includes: physical assessment, blood pressure, oxygen saturation, etc.
  • Review the medications you are taking
  • Arrange for any lab tests that may be needed
  • Assess your living conditions and how they may affect your health
  • If necessary, refer you to more specialized care programs

What People Are Saying

“We are so grateful for HouseCalls. They are always prompt with an answer or they get back to us quickly. Office staff are always pleasant, concerned, and friendly. If someone needs to be seen, it is arranged right away. It’s a blessing not to have to bundle the patient up for an offsite doctor’s visit, or find coverage for the other residents. Add in the other home services such as: blood draws, x-rays, physical therapists, and more, and HouseCalls is indispensable to us. We greatly value each member of your team! It makes being a caregiver easier. It’s great to know you’re only a phone call away. Obviously, you’re a group of highly dedicated people and we want you to know how grateful we are. Thank you so very much!” – Jan Oles, Owner, Oak Crest Adult Foster Care Home, Niles
